Description
Spanning the dorsum of the male hand, the dorsal metacarpal veins course longitudinally over the second through fifth metacarpals and interconnect as a dorsal venous network superficial to the extensor tendons. Distally, digital venules converge from each finger toward the intermetacarpal spaces, then unite proximally into larger channels that drain toward the dorsal venous arch near the bases of the metacarpals. From there, flow continues proximally and laterally toward the cephalic side of the forearm, with medial drainage contributing toward the basilic side. Surface anatomy is the point. Clinicians care about this posterior venous pattern because it is the default site for dorsal hand cannulation and peripheral IV starts, where superficial veins are accessible but prone to infiltration, thrombophlebitis, and patient discomfort if punctures traverse adjacent dorsal sensory branches. The metacarpal region also matters in traumatic swelling and dorsal hand infections, where venous congestion can obscure landmarks and make the dorsal venous arch less reliable for access.
