Description
Posterior to the metacarpals and extensor tendons, the superficial dorsal venous network (rete venosum dorsale manus) spreads as interconnecting venules across the dorsum of the male hand, then converges proximally into larger-caliber channels at the distal forearm. Digital veins course along the dorsal aspects of the fingers and unite near the metacarpophalangeal regions, where the network becomes denser and more transverse. Along the radial margin, tributaries typically align toward the cephalic vein, while ulnar-sided channels trend toward the basilic vein, with the dorsal venous arch bridging between them. Thin branches remain superficial, while accentuated vessels suggest deeper or more dominant drainage pathways at the wrist. Palpable, visible, and often cannulated, the dorsal venous network is a workhorse access site when antecubital veins are unsuitable, yet its mobility over the extensor apparatus makes stabilization and catheter securement a practical issue. Extravasation injuries on the dorsum of the hand are common with irritant infusions, and a clear map of the superficial pattern helps explain why swelling can track rapidly across metacarpal spaces. For surgeons, the same venous web sets the bleeding field for dorsal approaches to the metacarpals and phalanges, and it guides incision planning in tendon repair or open reduction where superficial veins are preserved to limit hematoma.
