Description
Seen from a posterior perspective, the liver occupies the superior right upper abdomen, with the gallbladder nestled on its inferior visceral surface and the extrahepatic biliary tree coursing medially toward the porta hepatis. Inferior and slightly medial to the liver, the C-shaped duodenum curves around the head of the pancreas, so the pancreatic head sits medial to the descending (second) part of the duodenum while the gland’s body extends leftward across the posterior abdominal wall. The common bile duct descends posterior to the first part of the duodenum and approaches the pancreatic head before entering the duodenal wall, aligning the biliary and pancreatic outflow pathways. That posterior relationship is the one surgeons and endoscopists work with. Obstruction at the distal common bile duct, most often from a stone migrating from the gallbladder or from a pancreatic head mass, produces conjugated hyperbilirubinemia and can precipitate pancreatitis when the pancreatic ductal system is involved at or near the hepatopancreatic ampulla. This view also clarifies why mobilizing the duodenum (Kocher maneuver) exposes the pancreatic head and distal bile duct during pancreaticoduodenectomy or trauma exploration.
