Posteriorly, the eleventh thoracic vertebra (T11) is presented in anatomical position with its spinous process projecting in the midline and the laminae forming the posterior arch on either side. The superior and inferior articular processes sit just lateral to the base of the spinous process, framing the vertebral canal, while the transverse processes extend laterally from the pedicle lamina junction. T11’s posterior elements begin to resemble the upper lumbar pattern, with a shorter, blunter, more horizontal spinous process than the long, inferiorly sloped spines typical of mid thoracic levels. This posterior perspective matters because T11 lies at a transitional junction where facet orientation and posterior element morphology shift toward lumbar mechanics, a point that influences segmental motion and surgical planning. Posterior exposure for thoracic decompression or instrumentation relies on confident identification of the spinous process, lamina, and facet joints, and T11 can be mistaken for T12 or L1 when the spinous processes lose the classic thoracic down-slope.
