Description
Arising at the cervicothoracic junction, the first thoracic vertebra (T1) is presented from a posterior perspective with adjacent lower cervical elements superior to it and the upper thoracic column continuing inferiorly. The T1 spinous process projects posteriorly and slightly inferiorly, typically thicker and less bifid than the cervical spinous processes above, while the laminae converge medially toward the midline. Lateral to the midline, the transverse processes extend to either side and bracket the vertebral arch, and the superior articular processes sit superior and posterior relative to the vertebral body to meet the inferior facets of C7. T1 matters because it marks the transition from the mobile cervical spine to the rib-bearing thoracic spine, a region where biomechanics and injury patterns change. Facet orientation shifts here, and that influences the spread of flexion-extension versus rotation. Clinically, the C7 to T1 level is a common site for degenerative spondylosis and can be difficult to visualize on lateral radiographs due to shoulder overlap, so posterior bony landmarks and the character of the spinous process help with level identification for reporting, injections, or posterior exposure. In teaching, this posterior view supports vertebral counting exercises in gross anatomy and spine modules, and it also fits well in radiology and orthopedic texts discussing cervicothoracic alignment, facet arthropathy, and posterior approach anatomy for laminectomy or instrumentation at the T1 level.
