Description
Posterior abdominal wall anatomy is presented with both kidneys (nephros) in situ, their renal parenchyma lying retroperitoneally on either side of the vertebral column. Superior poles sit deep to the distal eleventh and twelfth ribs, while the inferior poles descend toward the iliac crests, with the left kidney typically positioned slightly more superior than the right. Medial borders face the spine at the renal hilum region, where the renal artery and renal vein course to and from the abdominal aorta and inferior vena cava, and the proximal ureters begin their inferomedial descent. Segmental innervation is suggested by the prominent yellow nerve trunks and plexuses tracking along the posterior body wall adjacent to psoas major and quadratus lumborum. Posterior perspective matters when you want to teach why flank pain behaves the way it does and how posterior approaches reach the kidney without entering the peritoneal cavity. Percutaneous nephrolithotomy and nephrostomy access target a posterior calyx through the renal parenchyma, so relationships to the twelfth rib, pleura, and diaphragmatic attachments drive safe needle trajectory and help avoid supracostal complications such as pneumothorax.
