Centered in the posterior thoracoabdominal cavity, the abdominal aorta descends anterior to the vertebral bodies and gives rise to paired renal arteries that course laterally toward the renal hila on either side. Each kidney lies retroperitoneally, inferior to the lungs and diaphragm and posterior to the intestinal loops, with the right kidney positioned slightly more inferior because of the liver superiorly and anterolaterally. Running to the right of the aorta, the inferior vena cava receives the renal veins, which traverse medially from each hilum, with the left renal vein typically crossing anterior to the aorta on its way to the cava. Perihilar branching is suggested, with segmental distribution at the kidney’s medial border. Clean midline anatomy. Renal vascular anatomy is where normal variation meets high stakes.
