Description
Posteriorly, the dural venous system is centered on the midline occipital sinus, a slender venous channel within the attached margin of the falx cerebelli and along the internal occipital crest. It ascends from the region of the foramen magnum toward the confluence of sinuses (torcular Herophili), where it communicates with the transverse and superior sagittal sinuses and often with the straight sinus via the posterior aspect of the tentorium cerebelli. Inferiorly, continuity with the marginal sinus and internal vertebral venous plexus can be inferred at the craniovertebral junction, immediately posterior to the medulla oblongata and upper cervical spinal cord. Small but consequential. Clinical relevance concentrates at the posterior fossa and foramen magnum, where the occipital sinus becomes an operative and interventional hazard because its size is variable and it may represent a dominant midline drainage pathway when one transverse sinus is hypoplastic. Midline suboccipital craniectomy, posterior fossa decompression for Chiari I malformation, and approaches to the vermis and fourth ventricle all traverse tissues where this sinus may be encountered, and injury can produce troublesome venous bleeding or postoperative dural sinus thrombosis. Venous connections to the vertebral plexus also matter for posterior cranial fossa venous outflow patterns and potential pathways of infection or tumor spread. Neuroanatomy and head and neck anatomy courses can use this posterior perspective to teach dural reflections, venous sinus topology, and the relationship between intracranial venous channels and the upper cervical canal. It also fits well in atlases and surgical texts covering posterior fossa approaches, radiology teaching files discussing dural venous sinus variants, and patient education materials explaining the venous anatomy near the foramen magnum.
