Description
Running in the midline along the attached margin of the falx cerebri, the superior sagittal sinus forms a longitudinal venous channel from the region of the frontal crest and crista galli posteriorly toward the confluence of sinuses. Lateral to its walls, parasagittal venous lacunae spread between the meningeal layers, and granular foveolae (impressions from arachnoid granulations) indent the inner table of the calvaria adjacent to the sinus. Bridging veins from the superior cerebral veins approach the sinus from each hemisphere, crossing the subdural space to enter its lateral aspect along the superior margin of the longitudinal fissure. That parasagittal anatomy is where benign imaging findings and dangerous pathology sit side by side: arachnoid granulations can mimic filling defects on CT venography or MR venography, while true superior sagittal sinus thrombosis produces venous congestion, hemorrhagic infarction, and raised intracranial pressure. The relationship of bridging veins to the sinus also explains why rapid acceleration or deceleration can shear these vessels and lead to subdural hematoma, even when the cortical surface itself looks intact.
