Description
Centered within the cranial midline, the superior sagittal sinus runs posteriorly in the attached margin of the falx cerebri along the longitudinal fissure between the cerebral hemispheres. From this posterior perspective, its distal course approaches the internal occipital region, where it expands toward the confluence of sinuses (torcular Herophili) and communicates with the transverse sinuses along the tentorium cerebelli. Bridging cortical veins can be inferred entering the sinus along its parasagittal course, while the cerebellum and upper cervical spinal cord sit inferior to the occipital lobes at the craniovertebral junction. Dura matters here. A posterior view that emphasizes the termination of the superior sagittal sinus is the clearest way to teach venous outflow patterns at the torcular and their anatomic variability, a detail that affects everything from catheter navigation to operative planning. Neurosurgeons working near the midline occipital bone and posterior fossa (for example during craniotomy for falcine or parasagittal meningioma, or when exposing the transverse sinus) must respect the sinus walls and adjacent dural reflections to avoid torrential venous bleeding and postoperative venous infarction. This same region anchors clinical discussions of dural venous sinus thrombosis, where occlusion of the superior sagittal sinus or confluence can drive intracranial hypertension and hemorrhagic venous infarcts in parasagittal cortex.
