Removed calvaria exposes the male cranial cavity with the dural venous sinuses picked out in blue, including the superior petrosal sinus coursing along the superior border of the petrous part of the temporal bone at the attached margin of the tentorium cerebelli. Lateral to the dorsum sellae and posterior to the cavernous sinus region, this channel runs posteriorly toward the junction with the transverse sinus near the petromastoid area. Adjacent landmarks you would expect in this superior intracranial view include the anterior and middle cranial fossae, the orbital roofs and ethmoidal region anteriorly, and the internal occipital surface posteriorly. Bone first. Veins second. Tracing the superior petrosal sinus matters because it is the key dural conduit linking cavernous sinus outflow to the transverse-sigmoid system, so it sits directly in the operative corridor of posterior fossa and petroclival approaches and along the tentorial edge. Thrombosis or septic propagation from the cavernous sinus can alter venous drainage patterns and raise intracranial venous pressure, and the superior petrosal sinus often becomes the structure you must preserve, ligate, or work around when incising the tentorium. Its relationship to the petrous ridge also frames the course of nearby cranial nerves at the petrous apex, a frequent point of confusion in teaching.
