Viewed from the posterior aspect of an obese adult male in anatomical position, the thoracic viscera occupy the superior portion of the field, with the right and left lungs flanking the vertebral column and filling the posterior thorax. Inferior to the diaphragm, the retroperitoneal organs sit closest to the dorsal body wall, with the kidneys positioned bilaterally on either side of the spine, their superior poles lying deep to the lower ribs. More anterior abdominal viscera such as the liver, stomach, and intestinal loops are suggested through overlay, but they read as deeper and more ventral than the kidneys when approached from behind. Posterior organ topography matters most when you are teaching or planning access to the retroperitoneum, because the kidneys and adrenal (suprarenal) glands remain relatively fixed against the posterior abdominal wall even as central obesity increases abdominal depth and alters surface landmarks. This relationship underpins posterior and posterolateral approaches for nephrectomy and percutaneous nephrostomy, where the 12th rib, paraspinal musculature, and pleura form the corridor and the hazards. Obesity complicates these pathways by increasing skin to organ distance and shifting the working angles, while the kidneys remain tethered in the paravertebral gutters.
