Description
Anterior anatomy of an obese adult male torso is rendered with the body wall semi transparent, letting the respiratory tract sit in context against the chest contour and subcutaneous adipose tissue. Superiorly, the trachea descends in the midline from the cervical region into the superior mediastinum, then bifurcates at the carina into the right and left main bronchi, each entering its lung at the hilum. The lungs occupy the right and left pleural cavities laterally, their medial borders framing the mediastinal space where the cardiac silhouette would lie, though the heart itself is not shown. Inferiorly, the thoracic structures relate to the diaphragm and the upper abdominal viscera suggested by the title. Obesity changes what you can access and what you can image, and this view makes that obvious without distracting from airway and lung relationships. Extra anterior chest wall thickness and a lower resting diaphragmatic position can reduce functional residual capacity and worsen atelectasis risk under anesthesia, so the tracheobronchial tree and lung fields need to be understood in their thoracic envelope. Airway management hinges on landmarks. Carinal deviation, mainstem intubation (more often right), and aspiration patterns into the right lower lobe all make more sense when the bronchi are seen in situ from the front.
