Description
Framed from the posterior neck of an adult male, the posterior scalene is shown deep to the superficial posterior cervical musculature, arising from the posterior tubercles of the transverse processes of the mid to lower cervical vertebrae and descending inferolaterally toward the upper ribs. Occipital bone and posterior calvarial sutures sit superiorly, with the cervical spinous processes forming a midline bony reference as the muscle lies lateral to the vertebral column. Overlying landmarks include the trapezius spanning from the nuchal region toward the acromial end of the scapular spine, while the sternocleidomastoid is suggested more anterolaterally at the side of the neck. Inferiorly, the superior angles of the scapulae anchor the transition into the upper thorax. Posterior access clarifies why the posterior scalene is often the least intuitively understood of the scalene group in surface anatomy, yet it matters in thoracic outlet mechanics and in explaining lateral neck pain patterns when cervical transverse processes and first and second rib relationships are discussed. Its oblique fiber direction helps teach how neck muscles contribute to ipsilateral lateral flexion and assist in rib elevation during labored respiration. This depth relationship also sets expectations for procedural corridors, because blocks and approaches targeting the interscalene region must account for how far posterior the muscle belly sits relative to more superficial posterior cervical layers.
