Description
Running deep in the posterior compartment of the crus, the posterior tibial artery descends distal to the popliteal artery bifurcation, accompanied by venae comitantes and the tibial nerve as it courses toward the medial ankle. From a posterior perspective, its trajectory can be followed inferiorly behind the tibia and toward the region of the calcaneus (heel bone), where it enters the tarsal tunnel posterior to the medial malleolus. Superficial venous channels track in blue over the calf, while deeper paired veins run parallel to the red arterial trunks. Branching increases distally into the hindfoot and plantar circulation. Clinical relevance concentrates at the medial ankle. The posterior tibial artery is palpated just posterior to the medial malleolus, and loss or asymmetry of the posterior tibial pulse helps localize peripheral artery disease below the knee when dorsalis pedis findings are equivocal. In trauma and orthopedics, this vessel lies at risk during medial approaches to the ankle and calcaneus, and its position relative to the flexor retinaculum matters when distinguishing vascular compromise from tibial nerve entrapment in tarsal tunnel syndrome. Angiography figures, vascular surgery atlases, and podiatric and orthopedic teaching files routinely need a clear posterior map of the tibial, perimalleolar, and plantar vessels, paired with the superficial and deep venous networks for comparison. Use it for lower-limb anatomy labs, peripheral vascular disease lectures, or patient-facing explanations of pulse points and tarsal tunnel region procedures.
