Description
Dorsally, the second dorsal metacarpal artery is traced along the intermetacarpal interval between the second and third metacarpals, emerging from the dorsal carpal arch and running distally toward the web space and proximal phalanges of the index and middle fingers. Extensor tendons to the digits course superficially, with the extensor digitorum tendons and their dorsal digital expansions spanning the metacarpophalangeal joints, while the first dorsal interosseous muscle sits deep and lateral to the vessel near the second metacarpal. Metacarpal shafts and phalanges appear as pale osseous landmarks beneath the soft tissues. Orientation is unambiguous. A posterior view of this artery matters because dorsal metacarpal vessels form the main arterial substrate for many dorsal hand and finger flaps, and the second dorsal metacarpal artery is a common pedicle for reconstruction of dorsal digital defects of the index finger. Its relationship to the extensor apparatus and interosseous muscle also frames what goes wrong in dorsal hand trauma, where lacerations or crush injuries can compromise distal perfusion even when the palmar arches remain intact.
