Description

Dorsal metacarpal veins course superficially across the posterior aspect of the hand, forming a variable venous network overlying the metacarpals and extending distally toward the dorsal digital veins along the proximal phalanges. Proximally, these vessels converge toward the dorsal venous arch near the bases of the metacarpals, then ascend toward the radial and ulnar margins of the wrist to continue as the cephalic and basilic venous systems. Extensor tendons, including the tendons of extensor digitorum, extensor indicis, and extensor digiti minimi, run longitudinally deep to the superficial veins, with the intertendinous connections (juncturae tendinum) spanning between adjacent tendon slips. Intrinsic dorsal interossei occupy the intermetacarpal spaces beneath the venous plane. Superficial anatomy. Clear relationships. Dorsal metacarpal veins are frequent sites for peripheral IV cannulation, yet they can be small, mobile, and crossed by branches of the superficial radial nerve near the radial dorsum, a common source of painful paresthesia after cannulation or venipuncture. This view also supports teaching of dorsal hand swelling patterns and the spread of superficial infection or thrombophlebitis along the dorsal venous network toward the wrist.
