Description
Posteriorly oriented, the right and left lobes of the thyroid gland flank the trachea and are joined across the midline by the isthmus, with the posterior capsular surface in clear relief. Superior thyroid veins arise from the superior poles and course superolaterally, converging toward the upper segment of the internal jugular vein on each side of the carotid sheath. Small venules ramify over the gland surface, and the superior parathyroid glands are suggested as yellow ovoid bodies on the posterior aspect of the thyroid lobes. A compact relationship. Posterior thyroid venous anatomy matters in total thyroidectomy and in lateral neck dissection, where venous tributaries can be a primary source of bleeding during superior pole mobilization and when developing the plane near the carotid sheath. The superior thyroid veins provide a practical landmark for the cranial extent of thyroid dissection and for orienting the surgeon to the upper internal jugular vein, where inadvertent injury can lead to brisk hemorrhage or troublesome air entry in an open field. This view also reinforces why parathyroid preservation depends on recognizing posterior glandular attachments and the small vessels that accompany them.
