Description
Arising in the deep posterior compartment of the forearm, the posterior interosseous veins course proximally along the dorsal surface of the interosseous membrane, suspended between the radius laterally and ulna medially. Their paired venous channels track close to the posterior interosseous artery, threading beneath the extensor muscle mass from the distal forearm toward the elbow. More superficial dorsal veins of the hand and wrist sit posterior and distal to this deep system, while the bony landmarks of the radius, ulna, and carpal region anchor the relationships. Deep, narrow, and easy to miss. A posterior perspective is the cleanest way to teach how the deep venous drainage of the extensor compartment parallels the interosseous membrane rather than the more familiar cephalic and basilic pathways. This anatomy matters in posterior forearm approaches for fixation of radial and ulnar shaft fractures and in dorsal exposures for distal radius surgery, where deep vessels may be encountered with elevation of extensor musculature and retraction near the interosseous space. The same corridor is also where the posterior interosseous nerve runs, so appreciating vessel and nerve adjacency helps frame discussions of iatrogenic injury and postoperative hematoma. In upper limb anatomy courses, this plate supports teaching of deep versus superficial venous patterns and the organization of the posterior compartment around the interosseous membrane.
