Description
Rising along the midline, the posterior vertebral column is presented with the thoracic spinous processes forming a tight, inferiorly sloping ridge from the upper to lower thorax, then transitioning toward the broader lumbar spinous processes. Inferior to the thoracic region, the lumbar vertebrae show larger vertebral elements consistent with weight-bearing, and one lumbar vertebra is color-highlighted to isolate its posterior landmarks. From this perspective, the transverse processes project laterally from the vertebral arch while the spinous process remains centered and posterior. Orientation is unmistakably posterior. A posterior perspective matters because the thoracic spinous processes do not overlie their same-level vertebral bodies, a frequent source of counting errors when correlating surface landmarks with vertebral level for neuraxial anesthesia or thoracic epidural placement. The highlighted lumbar vertebra invites close attention to the posterior elements targeted during common procedures, including pedicle screw instrumentation, laminectomy, and medial branch blocks for facet-mediated low back pain. It also supports teaching of why thoracic rotation and lumbar flexion differ, given the change in facet joint orientation between regions.
