Description
Posteriorly, the thoracolumbar fascia spreads as a broad, tendinous aponeurosis across the lumbar and lower thoracic back, extending laterally toward the angles of the ribs and the iliac crest while remaining centered over the spinous processes. Its superficial lamina lies immediately deep to the skin and superficial fascia, blending inferiorly with the gluteal aponeurotic fascia and superiorly with the posterior thoracic fascia over the lower ribs. Medially, the fascial fibers interdigitate along the midline raphe over the lumbar vertebrae, and laterally they thicken into a dense sheet that provides attachment for the latissimus dorsi and interfaces with the external oblique and internal oblique aponeuroses. Layering matters here. This posterior perspective is the one you want when teaching how force transfer occurs between trunk and pelvis, because the thoracolumbar fascia forms a stiffened envelope around the paraspinal compartment while linking the latissimus dorsi superiorly to the contralateral gluteus maximus via fascial continuity. Clinically, it frames several pain generators and procedural planes: lumbar paraspinal compartment injections and fascial plane blocks rely on recognizing the thoracolumbar fascia as a distinct, palpable barrier, and ultrasound-guided thoracolumbar interfascial plane (TLIP) blocks target the fascial interval adjacent to the multifidus and longissimus to reduce postoperative pain after lumbar spine surgery.
