Description

Posteriorly, the brachial fascia forms a continuous sheath over the upper arm and thickens into a tricipital aponeurosis across the dorsal surface, investing the triceps brachii (long head medially, lateral head laterally, and the deeper medial head) while blending proximally with the posterior deltoid fascia and the fascia overlying teres major and latissimus dorsi. Lateral and medial intermuscular septa can be inferred as fascial partitions extending toward the humerus, separating the posterior compartment from the anterior arm flexors. Distally, this fascial envelope narrows toward the olecranon region, where the triceps tendon becomes the dominant midline structure and the fascia continues into the antebrachial fascia. Surrounding posterior trunk and lower-limb musculature remains visible as contextual anatomy in a full-body rear view. For teaching compartment anatomy, this posterior perspective clarifies what is meant by a closed myofascial space in the arm and why pressure rise in the posterior compartment behaves differently than superficial swelling. The relationships to the intermuscular septa matter in trauma and surgery: posterior humeral shaft fractures and operative approaches can place the radial nerve at risk as it traverses the spiral groove, tethered in part by septal and fascial planes. A tight fascial envelope also explains why deep hematoma around the triceps can present with pain on passive elbow flexion and loss of elbow extension strength.
