Description
Posterior anatomy of the male shoulder is rendered with the scapula and proximal humerus forming the glenohumeral joint, the humeral head seated lateral to the glenoid fossa and bordered superiorly by the acromion and posteriorly by the scapular spine. Osteophytes project near the base of the scapular spine, close to the spinoglenoid region where the spine meets the lateral scapular column, with adjacent cortical margins of the scapula and humerus clearly defined. A red periarticular halo marks local irritation around the joint line and posterior capsule. Bone change is the focus. Posterior osteophytes in this location most often reflect degenerative change from glenohumeral osteoarthritis or chronic instability, and they matter because even small bony excrescences can reduce capsular compliance and contribute to painful loss of external rotation and horizontal abduction. When spurring develops along the posterior glenoid rim or near the spinoglenoid notch, it can also narrow the corridor for the suprascapular nerve and irritate the infraspinatus branch, a pattern clinicians consider when posterior shoulder pain is paired with weakness in external rotation. The posterior view helps separate primary bony impingement from anterior causes of symptoms.
