Description

Oriented from a posterior abdominal approach, the colon is traced from the cecum and ascending colon on the subject’s right, across the transverse colon, then into the descending and sigmoid colon to the rectum and anal canal inferiorly. Running close to the mesenteric border, the superior mesenteric artery and inferior mesenteric artery give rise to the ileocolic, right colic, middle colic, left colic, and sigmoid arteries, which interconnect as marginal arterial arcades along the large intestine. Parallel venous channels accompany these branches, draining toward the superior mesenteric vein and inferior mesenteric vein, with the abdominal aorta and inferior vena cava centered posteriorly; both kidneys sit posterolaterally with renal vessels crossing to their hila. Posterior vascular anatomy of the colon matters when you are planning or teaching segmental colectomy and anastomosis, because the marginal artery (of Drummond) and the variable collateral pathways at the splenic flexure (a watershed region) often determine whether the remaining bowel has reliable perfusion. This perspective also reinforces why ligation level, such as high tie of the inferior mesenteric artery in oncologic left-sided resections, must be balanced against preservation of left colic flow and adequate venous outflow toward the portal system via the IMV and SMV.
