Description

Running superiorly from the posterior rectal wall, the superior rectal vein ascends within the mesorectum and tracks with the superior rectal artery toward the sigmoid colon and distal descending colon. Blue venous channels form a longitudinal arcade that lies posterior to the rectal ampulla and continues cranially as it approaches the inferior mesenteric venous system. Haustrated sigmoid colon transitions inferiorly into the straighter rectum, with the anal canal positioned most inferiorly in the field. Small perirectal lymph nodes sit along the vascular pedicle and within the surrounding mesenteric fat. Posterior orientation makes the mesorectal plane and the superior rectal vascular pedicle easy to teach, which is exactly where surgeons work during total mesorectal excision for rectal cancer and high ligation of the inferior mesenteric vessels. Venous drainage here has direct clinical consequences: portal hypertensive enlargement of the superior rectal venous tributaries contributes to internal hemorrhoids via the rectal venous plexus and portosystemic anastomoses with the middle and inferior rectal veins. Bleeding follows the vein, not the mucosa.
