Description
Oriented in lateral profile, the rectum and distal sigmoid colon sit posterior to the urinary bladder and prostate, with the pelvic peritoneal reflection turning onto the anterior rectal wall to form the rectovesical pouch. Running within the perirectal fascia (fascia propria of the rectum) and adjacent mesorectal fat, the middle rectal vein courses from the lateral rectal wall toward the pelvic sidewall, where it typically drains into the internal iliac venous system. Superior to it, tributaries of the superior rectal vein ascend with the superior rectal vessels toward the inferior mesenteric vein, while inferiorly the internal rectal venous plexus communicates with veins of the anal canal. Spatial relationships matter here. For pelvic surgeons, the middle rectal vein is a small structure with outsized consequences: it crosses the dissection plane during total mesorectal excision and can be a stubborn source of venous bleeding when the lateral ligaments of the rectum are divided. Its communications with the internal iliac territory also help explain why rectal cancer can spread beyond the portal route, and why venous drainage patterns differ between upper rectum and distal rectum. This lateral view supports teaching of the portal systemic anastomoses that underlie hemorrhoidal disease, while still respecting the distinct superior, middle, and inferior rectal venous pathways.
