Description
Posteriorly oriented cardiac lymphatics trace the epicardial surface along the coronary sulcus, with collecting vessels coursing between the left atrium and left ventricle and continuing toward the region of the atrioventricular groove. Superiorly, the great vessels are visible at the base of the heart, including the ascending aorta and pulmonary trunk, while the superior and inferior venae cavae enter the right atrium on the right side of the field. Coronary arteries and veins run in parallel on the surface, providing landmarks for the lymphatic channels that accompany them. Clear spatial relationships. Attention to the posterior collecting pathways matters because lymph from the myocardium does not drain uniformly, and surgeons working around the left atrium, pulmonary veins, and coronary sinus can encounter lymphatic collectors that contribute to postoperative edema or persistent pericardial effusions. This view also supports teaching of why malignancy and inflammation involving the pericardium or adjacent mediastinum can present with regional nodal enlargement along the tracheobronchial chain, given the heart’s lymphatic outflow toward mediastinal nodes rather than toward peripheral nodal basins. Coronary sulcus mapping helps.
