Description
Posterior anatomy of an adult African male is rendered with the large intestine overlaid in situ, allowing the colon to be read against the posterior abdominal wall. The ascending colon tracks superiorly on the subject’s right, continuing into the right colic (hepatic) flexure and transverse colon, which crosses the upper abdomen before turning at the left colic (splenic) flexure into the descending colon on the subject’s left. Inferiorly, the sigmoid colon curves medially within the left lower quadrant toward the midline and rectum. Retroperitoneal relationships are implied, with the posterior colon draped anterior to the kidneys and adjacent posterior structures, while the ascending and descending segments sit more fixed along the paracolic gutters. This posterior perspective is the clearest way to teach why the ascending and descending colon are secondarily retroperitoneal and relatively immobile, while the transverse and sigmoid colon remain intraperitoneal on a mesentery and move with distention or patient positioning. Surgeons rely on these planes when mobilizing the colon along Toldt’s fascia during colectomy, and radiologists use the same anatomic logic when localizing colonic dilation or pneumoperitoneum on CT in relation to the renal fascia and posterior abdominal wall.
