Description
Frontal anatomy of an African adult male torso is used as a surface map for the large bowel, with the ascending colon positioned on the patient’s right, the transverse colon crossing the upper abdomen, and the descending colon running inferiorly on the patient’s left. Superiorly, the hepatic flexure lies beneath the right hemidiaphragm and the costal margin, while the splenic flexure sits higher and more posterior near the left upper quadrant. The sigmoid colon curves in the left iliac fossa before continuing into the rectum in the midline pelvis. Haustra and the general caliber of the colon are represented along its natural course. An anterior perspective like this is the clearest way to teach regional localization of colonic pain, tenderness, and distension, tying quadrants and surface landmarks to the true intraperitoneal and retroperitoneal segments (ascending and descending colon typically retroperitoneal, transverse and sigmoid usually intraperitoneal with mesocolon). It also supports explanation of why the splenic flexure can act as a watershed zone in ischemic colitis (Griffiths point), and why the hepatic and splenic flexures are common sites of angulation that complicate colonoscope advancement. Expect these flexures to track beneath the diaphragmatic domes. They do. Use this artwork in gross anatomy and gastrointestinal blocks when introducing large intestine topography, and in patient-facing education for colonoscopy preparation, diverticular disease patterns, or correlating CT abdomen findings with physical exam.
