Description

Centered within the male abdominal cavity, the coiled jejunum and ileum occupy most of the infraumbilical and central quadrants, suspended by mesentery and bordered peripherally by the colon. Superiorly, the transverse colon crosses the abdomen beneath the epigastrium, while the ascending colon tracks along the right flank from the cecal region and the descending colon runs inferiorly on the left toward the sigmoid colon and rectum. Proximally, the duodenum begins in the upper abdomen, transitioning into the jejunum, and distally the ileum converges toward the ileocecal junction in the right lower quadrant. Framing the small bowel within the large bowel matters because it matches the mental map clinicians use for abdominal pain, obstruction patterns, and operative orientation. The anterior layout highlights classic sites of pathology, including cecal and sigmoid volvulus, obstructing colorectal carcinoma in the left colon, and Crohn disease frequently involving terminal ileum near the ileocecal valve. It also supports procedural planning, since colonoscopy follows this course and laparoscopic port placement often keys off the expected position of the cecum, transverse colon, and sigmoid loop.
