Description
Anteriorly oriented, a general frontal survey of the male intestine would place the duodenum in the right upper quadrant, its second and third parts curving around the head of the pancreas and crossing toward the left at the level of the L3 vertebral body. Centrally, jejunal loops typically occupy a more left upper to periumbilical position, while the ileum drifts inferiorly and rightward to end at the ileocecal junction. Peripheral framing comes from the large intestine, with the ascending colon on the patient’s right, transverse colon crossing superior to the small bowel mesentery, and descending colon on the left before continuing into the sigmoid colon in the left lower quadrant. Topographic relationships like these matter when correlating pain patterns and obstruction level on physical exam and imaging. A frontal layout helps clarify why duodenal obstruction or malrotation can present with bilious vomiting, and why terminal ileal disease, as in Crohn disease, often produces right lower quadrant tenderness near the ileocecal valve.
