Description
Seen from behind in an adult male, the upper cervical spine and occipital bone form the midline framework for the superficial and deep posterior neck musculature. Bilateral trapezius muscles span inferolaterally from the nuchal region toward the acromion and spine of the scapula, framing the medial borders of the scapulae. Deep to trapezius, the levator scapulae would be expected to course obliquely from the posterior tubercles of the transverse processes of C1 to C4 down to the superior angle and adjacent medial border of the scapula. Clear bony landmarks remain visible. This posterior perspective matters when you want to teach scapular position as a function of cervical and scapulothoracic mechanics. Tightness or overactivity of levator scapulae commonly contributes to ipsilateral cervical side-bending with scapular elevation and downward rotation, a pattern often reported as medial superior scapular pain in desk-based workers and cyclists. It also helps clarify why the dorsal scapular nerve (C5) and dorsal scapular artery travel deep to levator scapulae and the rhomboids, relevant when local injections, trigger-point needling, or surgical dissection is performed along the medial scapular border.
