Description
Running from the transverse processes of C1 to C4 down to the superior angle and medial border of the scapula, the levator scapulae occupies the posterolateral neck where it lies deep to the upper trapezius and posterior to the sternocleidomastoid. Medially, its cervical attachments sit lateral to the spinous processes and deep to splenius capitis and splenius cervicis, while laterally the muscle approaches the posterior triangle toward the scapular girdle. Inferiorly, its scapular insertion is just superior to the root of the scapular spine, positioned medial to the supraspinous fossa and proximal to the acromion. Bony landmarks anchor the orientation. That posterior perspective matters when you are teaching how cervical motion couples to scapular position, because levator scapulae elevates and downwardly rotates the scapula while also contributing to ipsilateral cervical sidebending and rotation when the scapula is fixed. Palpation and injection planning often target the muscle belly near the superior angle of the scapula, a common trigger point generator in cervicogenic headache and postural neck pain. Its innervation by the dorsal scapular nerve (C5) and cervical roots makes it a practical structure for correlating upper cervical symptoms with scapular dyskinesis, and for discussing compensatory overactivity when trapezius function is reduced after spinal accessory nerve injury.
