Originating from the transverse processes of C1 to C4, the levator scapulae descends inferolaterally along the posterolateral neck to insert on the superior angle and medial border of the scapula. In lateral profile, its belly sits deep to the sternocleidomastoid, which runs more anteriorly from the mastoid process to the manubrium and medial clavicle. Posterior to the sternocleidomastoid, the levator scapulae tracks toward the shoulder girdle and blends visually with adjacent posterior cervical musculature as it approaches the scapula. Bony context from the skull and auricle helps anchor the cervical region in true anatomical position. This lateral relationship matters in the clinic because palpable neck pain often reflects tension in the levator scapulae rather than sternocleidomastoid strain, yet the two are easily conflated on surface exam.
