Description
Posterior to the distal humerus, the middle collateral artery descends from the profunda brachii (deep brachial) artery within the posterior compartment, tracking inferiorly toward the elbow joint. It courses adjacent to the triceps brachii, then approaches the olecranon region where it participates in the periarticular anastomosis with the recurrent interosseous artery and ulnar collateral branches; the brachial artery continues anteriorly before bifurcating into the radial and ulnar arteries in the cubital fossa. Superficial venous channels, including the cephalic vein laterally and the basilic vein medially, run in the subcutaneous tissue and communicate across the elbow via the median cubital vein. A dense network surrounds the olecranon. This posterior vascular view matters when you need to teach or plan around the elbow’s collateral circulation, which maintains distal perfusion when the brachial artery is kinked in flexion, compressed, or temporarily occluded. The middle collateral artery is also a relevant landmark in posterior approaches to the distal humerus and elbow (triceps-splitting or paratricipital intervals), where iatrogenic bleeding can obscure the operative field and where soft tissue stripping can jeopardize the anastomotic ring. Elbow trauma and distal humeral fracture fixation routinely challenge these vessels.
