Description
Seen from a posterior aspect in an adult African male, the paired kidneys sit retroperitoneally on either side of the vertebral column, with the right kidney positioned slightly inferior to the left. Each renal pelvis narrows into a ureter that descends vertically along the posterior abdominal wall, remaining medial to the lower poles before entering the pelvis. The urinary bladder lies midline in the lesser pelvis, inferior to the peritoneal cavity, and the urethra continues distally from the bladder neck. A posterior orientation clarifies relationships that matter in practice: the kidneys lie anterolateral to the thoracolumbar spine and deep to the paraspinal and posterior abdominal wall musculature, so posterior flank pain patterns and percutaneous access routes make anatomical sense. Percutaneous nephrostomy and percutaneous nephrolithotomy depend on appreciating the kidney’s posterior surface and calyceal axis while avoiding pleural violation near the superior pole and limiting vascular injury by targeting a posterior calyx. The long axis of the descending abdominal aorta, tracked against the vertebral bodies, reinforces why aneurysmal dilation or dissection can refer pain to the back and why midline vascular structures remain anterior to the spine even when approached conceptually from behind.
