Posterior to the knee joint, the popliteal artery courses through the popliteal fossa as the continuation of the femoral artery after it exits the adductor hiatus, then descends toward the inferior border of the popliteus where it approaches its terminal division into the anterior tibial artery and the tibioperoneal trunk. The popliteal vein accompanies it in close apposition and typically lies superficial (posterior) to the artery in this corridor, while the femoral condyles sit superiorly and the tibial plateau inferiorly as osseous landmarks framing the vessel’s path. Articular cartilage caps the femoral condyles and tibial surfaces, clarifying the relationship of the neurovascular bundle to the posterior capsule. Short and direct. This posterior perspective matters because the popliteal artery is fixed proximally at the adductor hiatus and distally at the soleal arch region, a tethering that helps explain its vulnerability in posterior knee dislocation and displaced tibial plateau fractures, even when distal pulses seem preserved. Surgeons rely on these relationships during posterior approaches to the knee and when planning popliteal artery exposure for bypass, thrombectomy, or repair of a traumatic intimal tear, where the vein’s usual posterior position increases the risk of venous injury during arterial control. The broad pre-trifurcation segment is a key landmark for catheter-based interventions and for localizing popliteal artery entrapment relative to the popliteus and adjacent musculotendinous structures.
