Description

Anterior thoracic anatomy is presented with both breasts overlying the superficial fascia and pectoral region, centered on the nipple and areola complex and extending from the parasternal line medially toward the anterior axillary line laterally. Lobulated glandular parenchyma and lactiferous ducts converge on the nipple, suspended by fibrous Cooper ligaments within predominantly adipose tissue. Deep to the mammary tissue, the pectoralis major forms the principal muscular bed, with perforating vessels and superficial veins coursing toward the axilla and sternum. Cutaneous sensory innervation is represented along the course of the anterior and lateral cutaneous branches of the intercostal nerves. An anterior view like this matters when you need to teach the relationship between breast parenchyma and the pectoral fascia, because that plane defines the classic surgical dissection for mastectomy and influences patterns of tumor spread. Attention to vascular supply helps orient the internal thoracic (medial mammary) and lateral thoracic contributions that can complicate reduction mammaplasty or flap planning, and the axillary tail of Spence explains why lesions may present high and lateral on the chest wall. Palpation landmarks become easier to explain. So does why lymphatic drainage targets the axillary nodes and, secondarily, parasternal nodes.
