Description
Oblique left anterior anatomy centers on the anterior interventricular artery (left anterior descending, LAD) descending in the anterior interventricular sulcus toward the apex, with a prominent diagonal branch coursing superolaterally across the anterior surface of the left ventricle. The left atrial appendage sits superior and leftward, partially overlapping the proximal left coronary territory, while the pulmonary trunk lies anterior to the ascending aorta at the base. Epicardial fat and the visceral pericardium frame the coronary vessels on the myocardium, and accompanying cardiac veins run in parallel grooves, oriented more superficially within the epicardium. Diagonal branches are a constant teaching and clinical target because they supply the anterolateral left ventricular wall, a territory that often drives anterior ischemia patterns when stenosed, and they can be the culprit in ST elevation involving I, aVL, and the anterior precordial leads depending on dominance and branch size. This angle is also the one many operators mentally reconstruct during coronary angiography and percutaneous coronary intervention, where the takeoff of the diagonal from the LAD and its relationship to septal perforators influences wiring strategy and bifurcation stent planning.
