Description
Oriented from the diaphragmatic (inferior) surface, the left ventricle dominates the field with the inferior left ventricular branch arising from the circumflex artery and coursing inferiorly toward the apex along the posterior wall. The circumflex artery tracks in the left atrioventricular (coronary) sulcus, lateral to the left atrium and superior to the ventricular myocardium, before giving off ventricular branches that run distal over the epicardial fat. Adjacent coronary veins accompany the arterial tree in the sulci, and the right ventricle and posterior interventricular region provide medial reference for the inferior surface anatomy. Pericardial contours and the basal great vessel roots at the superior margin frame the coronary distribution. This specific branch matters when mapping perfusion of the inferior and posterolateral left ventricular myocardium, a territory often implicated in ischemia from left circumflex disease and, in left-dominant circulation, larger infarct patterns. The inferior view also places the vessel in the same orientation used to explain posterior myocardial infarction on ECG (reciprocal anterior changes) and to correlate wall-motion abnormalities on echocardiography and cardiac MRI. Small caliber arterioles and epicardial branches become practical landmarks when discussing collateral flow and the limits of revascularization in chronic total occlusion.
