Presented from an inferior (diaphragmatic) perspective, the heart is oriented with the apex directed inferiorly and the base superiorly, while the atrioventricular (coronary) sulcus is traced by the circumflex branch of the left coronary artery as it courses leftward and posteriorly between left atrium and left ventricle. From this circumflex artery, multiple atrial branches sweep superiorly over the posterior and lateral walls of the left atrium, running in the epicardial fat deep to the visceral pericardium. Nearby surface landmarks you would expect in this view include the posterior interventricular sulcus approaching the crux cordis, adjacent venous channels converging toward the coronary sinus, and the great vessel ostia at the base (aorta and pulmonary trunk) seen as circular openings. An inferior view is where the circumflex artery’s relationship to the left atrium becomes easiest to teach, because the atrial branches often arise at acute angles and disappear quickly into the atrial epicardium, making them easy to miss in standard anterior presentations. This anatomy matters in left atrial procedures, including mitral valve surgery and atrial fibrillation ablation, where atrial ischemia or bleeding can follow injury to circumflex atrial branches along the atrioventricular groove or near the posterior left atrial wall. Coronary dominance also comes into play at the crux, where circumflex continuation may contribute to the posterior descending artery and alter the expected supply of the inferior myocardium.
