Description
Oriented from an inferior (diaphragmatic) perspective, the right coronary artery (RCA) is traced along the right atrioventricular groove as it curves from the aortic root toward the crux cordis. Small atrial branches arise proximally and sweep superiorly over the right atrial wall toward the right auricle, running superficial to the myocardium and beneath the visceral pericardium (epicardium). Portions of the right ventricle and the diaphragmatic surface of the left ventricle frame the course of the RCA, while the inferior vena cava and the region of the coronary sinus sit posterior and medial in the same field. Attention to these atrial branches matters because the proximal RCA also gives rise to the sinoatrial nodal artery in many hearts, and distal RCA branches frequently contribute to the atrioventricular nodal supply near the crux. Ischemia in this territory helps explain bradyarrhythmias and varying degrees of AV block during inferior wall myocardial infarction, and it clarifies why ST elevation in II, III, and aVF can accompany conduction disturbance when a dominant RCA is occluded. The inferior view also supports procedural planning, since epicardial fat and the course of the right atrial vessels influence surgical dissection planes during right atriotomy, tricuspid valve exposure, and some approaches to atrial flutter ablation.
