Thyroarytenoid Muscle, Superior View

Description

Seen from a superior laryngeal perspective, the thyroarytenoid muscle (including its medial vocalis fibers) runs anteroposteriorly from the inner surface of the thyroid cartilage toward the arytenoid cartilage, forming the lateral muscular wall of the glottic aperture. Medial to the thyroarytenoid, the vocal ligament defines the free margin of the true vocal fold, while the muscle belly lies lateral and slightly inferior to that ligament along most of its course. Posteriorly, the thyroarytenoid abuts the vocal process region of the arytenoid, and laterally it relates to the paraglottic space and the inner perichondrium of the thyroid lamina. Small. Precise. This relationship matters because the thyroarytenoid is the primary “body” of the vocal fold and directly shapes phonation by shortening and relaxing the vocal ligament while fine-tuning tension through the vocalis. Superior views clarify why edema, scarring, or focal atrophy within the thyroarytenoid alters mucosal wave mechanics, a common finding in presbyphonia and after intubation-related injury. The same anatomy guides injection laryngoplasty and medialization procedures, where material placed too lateral in the thyroarytenoid can stiffen the fold, and placement too superficial risks affecting the vibrating cover.
