Description
Viewed from above into the laryngeal inlet, the paired arytenoid cartilages sit posterior to the true vocal folds, with the oblique arytenoid muscles crossing superomedially from the muscular process of one arytenoid to the apex of the contralateral side. Their decussating fibers continue superiorly as the aryepiglottic muscle within the aryepiglottic fold, which runs from the arytenoid region toward the lateral margin of the epiglottis. Anterior to this posterior laryngeal framework, the epiglottis and tongue base define the anterior boundary of the aditus, while the piriform recesses would lie laterally beyond the aryepiglottic folds. Orientation is immediate. Functionally, oblique arytenoid contraction draws the arytenoid cartilages toward one another and helps narrow the laryngeal inlet, a key component of protective sphincteric closure during swallowing and cough. This superior perspective also clarifies why supraglottic edema or scarring along the aryepiglottic folds can worsen airway obstruction out of proportion to the size of a lesion, and why laryngeal mask or endotracheal instrumentation often produces posterior supraglottic discomfort near the arytenoid complex. For laryngologists, the relationship between the arytenoid apices and the aryepiglottic folds is a practical landmark when correlating endoscopic findings with the underlying intrinsic laryngeal musculature.
