Across the anterior skull and orbits, the superior oblique muscles are shown running from their origin at the body of the sphenoid, just superomedial to the optic canal, toward the anteromedial orbit. Each muscle belly lies superior to the medial rectus and medial to the superior rectus before narrowing into a tendon that passes through the fibrocartilaginous trochlea on the frontal bone at the superomedial orbital rim. After redirecting, the tendon courses posterolaterally beneath the superior rectus to insert on the sclera posterior to the equator in the superolateral quadrant. Bone context includes the frontal bone, nasal bones, maxilla, mandible, and the cervical vertebrae at the craniovertebral junction. A small pulley, big consequences. This anterior perspective clarifies the trochlea as the key landmark that changes the superior oblique’s line of pull, producing intorsion with secondary depression in adduction, a pattern tested directly in motility exams and the Bielschowsky head-tilt test.
