Description

Arising from the dorsal midbrain just inferior to the inferior colliculus, the trochlear nerve (cranial nerve IV) courses posteriorly and decussates within the anterior medullary velum before winding laterally around the midbrain in the ambient cistern. From this lateral perspective, its thin fascicles are seen hugging the posterolateral surface of the cerebral peduncle, then running anteriorly toward the cavernous sinus and superior orbital fissure. Within the orbit, the nerve targets the superior oblique muscle, whose tendon passes through the trochlea at the superomedial orbital rim. Small nerve caliber. Big spatial lesson. That posterior emergence and midline crossing are the defining quirks of CN IV and a frequent point of confusion in neuroanatomy. Lesions along the subarachnoid segment, including traumatic stretch at the tentorial edge or compression near the posterior clinoid region, produce an ipsilateral superior oblique palsy with hypertropia and excyclotorsion, classically worse on contralateral gaze and ipsilateral head tilt (Bielschowsky head tilt test). A lateral view makes the nerve’s relationship to the midbrain and tentorial corridor easier to teach when correlating diplopia patterns with brainstem localization.
