Description
Running over the posterior iliac crest, the superior cluneal nerves (lateral cutaneous branches of the dorsal rami, typically L1 to L3) arc laterally and inferiorly to cross the crest and enter the superficial tissues of the upper buttock. Their key feature in this lateral perspective is the point where they pierce the thoracolumbar fascia near the iliac crest, then disperse as small sensory branches across the superolateral gluteal region. Gluteus maximus forms the dominant superficial muscular mass posteriorly, while gluteus medius and minimus sit more superior and deep, with tensor fasciae latae and the iliotibial tract occupying the lateral thigh. Clear landmarks matter. Entrapment of the superior cluneal nerves at the osteofibrous tunnel created by the thoracolumbar fascia and iliac crest is a recognized cause of low back pain and buttock pain that can mimic lumbar radiculopathy or sacroiliac joint dysfunction.
