Description
Seen from a lateral perspective in an adult male pelvis, the superior rectal nerve descends from the inferior hypogastric plexus toward the proximal rectum, tracking along the posterior and lateral contour of the rectosigmoid junction. The sigmoid colon is identifiable by its sacculated haustra and the longitudinal taeniae coli on the external surface, with the nerve course shown in relation to accompanying vessels within the mesocolon and superior rectal mesentery. Small nodular elements along the mesenteric border are consistent with pericolic lymph nodes or epiploic fat, positioned lateral to the bowel wall. Orientation is clear: the pelvic cavity lies inferiorly, and the nerve runs distally toward the superior rectum. This angle matters when you need to teach or plan around pelvic autonomic pathways that are vulnerable during rectosigmoid mobilization. Injury to sympathetic fibers carried via the superior rectal nerves and adjacent hypogastric plexus can contribute to postoperative bowel dysmotility and pelvic autonomic dysfunction after low anterior resection, sigmoid colectomy, or total mesorectal excision. The relationship to the superior rectal vessels and mesenteric planes is the surgical point, because nerve preservation depends on staying in the correct fascial layer while ligating the inferior mesenteric and superior rectal branches.
