Description
Seen from a lateral perspective in an adult male pelvis, the superior rectal nerve descends along the posterior aspect of the sigmoid colon toward the upper rectum, blending into the autonomic nerve plexus on the bowel wall. Fine branches spread over the serosa in parallel with mesenteric vessels, with epiploic appendages and haustra marking the transition from sigmoid colon to rectal segment. Proximally, the nerve fibers relate to the inferior mesenteric plexus and its ganglionated elements, then course distally toward the superior rectal vascular pedicle. This is the key autonomic pathway for the hindgut distal to the left colic flexure, and a clean lateral view clarifies how sympathetic fibers reach the rectum and how periarterial plexuses accompany the superior rectal artery. It matters in total mesorectal excision and low anterior resection, where dissection too close to the inferior mesenteric artery origin or the sacral promontory can injure hypogastric nerves and contribute to postoperative bowel dysfunction and sexual side effects.
