Description
Arising from the posterior divisions of the L4 to S1 ventral rami, the superior gluteal nerve exits the pelvis through the greater sciatic foramen superior to the piriformis, then courses laterally between gluteus medius and gluteus minimus. From this lateral perspective, its short trunk and rapid division into superior and inferior branches are appreciated as they track toward the deep surface of gluteus medius, with the adjacent sciatic nerve descending inferiorly toward the posterior thigh. The bony landmarks frame the pathway, including the iliac wing and acetabular region of the hip bone, the proximal femur with femoral head seated in the acetabulum, and the lumbosacral spine with intervertebral discs. Damage to the superior gluteal nerve produces a classic abductor mechanism deficit, with weakness of gluteus medius and minimus and a Trendelenburg gait or contralateral pelvic drop during single-leg stance. This view suits teaching and surgical planning because it emphasizes the nerve’s corridor above piriformis and its intimate relationship to the greater trochanteric region, where lateral and anterolateral hip approaches, retractor placement, and intramuscular injections can place the nerve at risk. Small nerve, big consequence. Use this asset in gross anatomy and neuroanatomy curricula when tracing sacral plexus outflow, and in orthopedic or sports medicine materials discussing postoperative limp after total hip arthroplasty, gluteal tendon repair, or pelvic fracture patterns that threaten the superior gluteal neurovascular bundle.
